Abstract
Mapping Your Community's Health Track: Health and Human Services Author(s): Andrew Bazemore, Shiloh Turner, Mark Carrozza Health Landscape is an interactive Web atlas that educates health professionals, policy makers, academic researchers and planners on how to geographically combine, analyze and display information in ways that promote understanding and improvement of health and healthcare. There is no cost to use the site. Health Landscape is a collaborative effort of the American Academy of Family Physicians, the Robert Graham Center, the Health Foundation of Greater Cincinnati, and the University of Cincinnati. There are three components to the site: Community Health View, Primary Care Atlas, and Health Center Mapping Tool. This session will describe each module as an innovative platform that public health and social service providers can use to identify needs in their community, evaluate their programs' effectiveness, and influence policy change.
