In a collaborative effort between multiple organizations, a data display and mapping tool has been created to assess how a North Carolinas communities are performing according to data related to health outcomes by allowing a variety of health indicators to be visually compared to key performance indicators and quality measures derived from Medicare and Medicaid cost and utilization data. In this online data display tool, users are able to dynamically display data from a variety of sources at the hospital referral region, county and Census Tract, and compare them to other indicators in side by side maps, or overlaying the data to look at counties with a combination of "high" and "low" classifications of variables as defined by the user. Additionally, members of the networks are also able to display the data at the individual practice level in order to compare those to the communities and counties that they serve.
