The Community Context of Health Center Patients

This analysis reports on patients and the community in which they live, leveraging access to population-based big data and geospatial technologies, allowing us to understand the social & environmental influences on the health of a physician’s patient panel. As patient vital signs provide a biometric snapshot of an individual’s health status, community vital signs from aggregated population health data and information collected directly from patients provide an environmental health perspective.
