Health behavior surveillance helps prevent and mitigate chronic disease in communities. We created ranked behavior profiles for nutrition, exercise, smoking, medical services use, and other factors using census tract-level market potential data. Medication use data is used to monitor diagnosed disease prevalence in areas too small to calculate stable medical encounter rates. These data help inform outreach and allow us to anticipate future healthcare challenges.
