Enter the ‘Wellness Map”, the first health map based on patient’s unique medical record considering the social determents of care. It's generated by harnessing the power of EHR and GIS systems and presents a wealth of community resources based on the patient's conditions. This person-centric approach helps physicians, patients and members of the community, to understand the wellness assets and, therefore, empower them to maintain a healthy, active lifestyle and avoid preventable health episodes.
