The Community Vital Signs API: A Novel Tool for Addressing SDH

Drawing on Institute of Medicine recommendations, we have created a secure “Community Vital Signs” API, which connects patients to the broader community. Using addresses from an originating data system, the API geocodes, assigns longitude and latitude, appends geographic identifiers, and integrates comprehensive community vital signs with the assigned geographic identifiers. We discuss ongoing evaluations of the utility of the Community Vital Signs API in both research and clinical settings.
